Epimedium, commonly known as horny goat weed, is a traditional plant used primarily in East Asian medicine, particularly in China. It has been traditionally employed for various purposes including sexual dysfunction and male infertility, though specific uses vary among different traditions. Scientific evidence suggests that icaritin from Epimedium may exhibit anti-fibrotic activity through multiple mechanisms, and Epimedokoreanin B shows potential in suppressing bacterial viability and virulence, which could be beneficial in managing periodontitis-related bone loss. Additionally, ICA (icariin) demonstrates promise in osteoporosis therapy by modulating osteogenic, anti-inflammatory, and angiogenic pathways; however, clinical translation is limited due to poor oral bioavailability. Dietary supplementation with Epimedium ultrafine powder has been observed to improve semen quality and hormonal levels in Bama boars through gut microbiota and metabolite modulation. No significant safety concerns or recorded drug interactions have been identified for this plant.
